Yes! Providing you follow the process set up through the Care Act 2014.
If a person is a self-funder, i.e. responsible for paying their care fees, then you will not need to contact the Local Authority.
If the Local Authority partly or wholly funds them, you must follow all steps below.
If possible, give up to 12 weeks’ notice to the Local Authorities.
Moving to a care home in a different area typically involves contacting the local council in the new area to arrange a care needs assessment and to discuss funding arrangements. Your current local council will then liaise with the new council and the care home to ensure a smooth transition, including continuity of care and support.
Here’s a more detailed breakdown:
- Inform the Existing Local Authority as well as the New Local Authority:
Contact the council’s social services department in both the current and new areas and inform them that you will require care.
- Care Needs Assessment:
Your current local council will conduct a care needs assessment to determine the level of support required.
- Funding Arrangements:
If eligible for funding, the current local council will be responsible for covering the cost of the care home placement in the new area, up to a specified limit.
- Liaison with the New Local Authority:
Your current council will contact the new council to arrange for the transfer of your care plan and other relevant information, ensuring continuity of care.
- Care Home Placement:
The new local authority will work closely with the care home to ensure that it can meet your individual needs.
- Smooth Transition:
The process aims to ensure a smooth transition, with no gaps in care and support during the move.
Important Considerations:
- Funding:
If you are self-funding, you will need to review your contract with your current care home and understand the process for terminating your agreement.
- Individual Needs:
Ensure the new care home meets your needs, including any medical requirements or preferences.
- Accessibility and Support:
Consider the practicality of visits from loved ones and the availability of local services in the new area.
- Continuity of Care:
Ensure your existing care plan and any relevant support services are transferred to the new care home.
